GRADE 4 ACNE: Extensive infected cystic acne / acne conglobata. The face and neck may be severely involved, with extensive lesions on the upper trunk. Possible extension into the scalp, and on the posterior neck, may be noted. Coalescence of cystic acne lesions occurs, with production of boggy canalized sinuses. Scarring may result in distorted borders, with cord-like bands and hypertropic ridges.
