The earliest known acne grading system was published in 1956 by Pillsbury, Shelley and Kligman. The Pillsbury grading score is one of several grading systems used to classifying different types of acne. The Pillsbury grading score can be found in the Pillsbury Textbook of Dermatology. Over the years, it has been modified in various ways by different practitioners to better accommodate their scope of treatment and acne treatment protocols. The following is based on the Pillsbury classification:

GRADE I ACNE: Comedones and occasional small cysts confined to the face.

GRADE II ACNE: Comedones with occasional pustules and small cysts confined to the face.

GRADE III ACNE: Many comedones and small and large inflammatory papules and pustules, more extensive but confined to the face.

GRADE IV ACNE: Many comedones and deep lesions tending to coalesce and canalize, and involving the face and the upper aspects of the trunk.
