In women with PCOS, a follicle's developing egg within an ovary may not fully mature. Instead, a cyst is formed containing the immature egg. When the egg does not mature, the follicle does not produce progesterone. Progesterone is the hormone responsible for the monthly shedding of the uterus (specifically the endometrial lining), and without it, a woman's menstrual cycle becomes irregular, sometimes absent. Without progesterone, the lining of the uterus can become thick, which can lead to heavy and/or irregular bleeding.
